The peroneal tendon is located just behind the outside portion of the ankle bone known as the lateral malleolus. From here the tendons run down the outside, or lateral aspect, of the foot and even underneath. Anywhere along this tendon pain can become apparent from inflammation creating tendonitis.

 

Some of the more common locations I have seen peroneal tendonitis develop are:

  • Just behind the ankle bone (the lateral malleolus)
  • Around the boney portion about 2 inches from the end of the 5th toe (styloid process of the 5th metatarsal).

The reason I wanted to put some information and a video out on the topic of peroneal tendonitis is because many times I see athletes come in under the impression their pain is from a stress fracture or locations of old fractures. Understandably so however, since a common cause of peroneal tendonitis can be old inversion ankle sprains, which can also cause minor fractures in the area.


 

The last client I had with peroneal tendonitis presented in this fashion:

  • Current indoor Volleyball player
  • 7 month old ankle sprain during a game
  • Inability to walk at time of injury due to fracture of lateral malleolus
  • Current pain behind the lateral malleolus with jumping and squatting
  • Unable to play at full speed

 

Under care she was able to play at full speed with a 3-week period of time with only minor tightness in the region during squatting.

 

To properly have your ankle/ foot condition diagnosed it is important to see a medical provider experienced in dealing with athletic injuries. However, before you go in you should be aware of exact locations of pain/ tightness when you are describing your condition to help the provider with a correct diagnosis.

 

Proper treatment and rehab of peroneal tendonitis usually should allow the athlete to play near full speed within the month, but it is important to emphasis to each athlete the possibility of minor flare-ups of pain or tightness from peroneal tendonitis as activity/ intensity increases. Usually I tell my athletes to expect coming back for minor “hands-on” treatment as the season continues, however they would be expected to continue with strengthening and balance-based rehab continually until we have reached 100% resolution.

 

Now to the treatment approaches I feel work well for peroneal tendonitis…
